Anuco Home Health Care LLC is seeking compassionate Occupational Therapists to provide patient-centered rehabilitation in patients’ homes around Chicago, IL. You will assess function, plan care, and deliver skilled therapy while supervising assistants and aides.

The ideal candidate has Medicare home health experience, strong clinical judgment, and proficiency with Axxess EMR, with a track record of accurate documentation and safe home-based interventions.

Qualifications

• Current, unrestricted Illinois Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• NBCOT certification or eligibility.
• Home health experience preferred.
• OASIS experience preferred.
• Axxess Home Health EMR experience preferred.
• Two years of occupational therapy experience preferred.
• CPR or BLS certification.
• Valid driver’s license and reliable transportation.

Responsabilités

• Complete comprehensive occupational therapy evaluations and reassessments.
• Evaluate functional mobility, strength, coordination, cognition, safety awareness, and ADL/IADL ability.
• Develop and revise individualized plans of care.
• Perform skilled occupational therapy procedures per physician orders.
• Supervise assigned COTAs and HHAs.
• Communicate patient progress and treatment changes to physicians and team.
• Instruct patients and caregivers on home program safety and continued care.
