Job summary

Anuco Home Health Care LLC is seeking a compassionate Certified Occupational Therapy Assistant to provide in-home occupational therapy under the supervision of a licensed OT in Chicago, IL and surrounding communities.

The role requires strong communication, time management, and documentation skills, with emphasis on adherence to plans of care and timely progress notes in the Axxess EMR system. Home health experience is preferred, and travel to patient homes is expected.

Qualifications

• Current, unrestricted Illinois OTA license.
• Graduate of an accredited OTA program.
• Current NBCOT certification as a COTA.
• Home health experience strongly preferred.
• Axxess Home Health EMR experience preferred.
• Two years of OTA experience preferred.
• Current CPR or BLS certification.
• Valid driver’s license, reliable transportation, and current automobile insurance.

Responsibilities

• Provide OT treatments delegated and supervised by the OT.
• Follow physician orders and plan of care established by the supervising OT.
• Assist patients with ADL/IADL, mobility, endurance, and daily activities.
• Provide ADL/IADL retraining and home-safety education.
• Educate patients and caregivers on home programs and adaptive techniques.
• Coordinate OT services with supervising OT and interdisciplinary team.
• Document treatment, patient response, and progress toward goals.
• Report changes in condition or safety concerns to the supervising OT.
