Job summary

Anuco Home Health Care LLC is seeking compassionate Certified Occupational Therapy Assistants to provide in-home occupational therapy under supervision of a licensed OT. The role emphasizes ADL/IADL retraining, home safety education, and coordinated care for patients in Chicago and surrounding communities.

Ideal candidates will have home health experience, strong communication, and proficiency with Axxess Home Health EMR, with documentation completed within required timeframes.

Qualifications

• Current Illinois OTA license required and active CPR/BLS certification.
• Graduate of an accredited OTA program with documented clinical competencies.
• Home health experience preferred and ability to travel to patient homes.

Responsibilities

• Provide occupational therapy treatments under supervising OT.
• Follow physician orders and the plan of care for patients at home.
• Assist with ADL/IADL retraining and safe home environments.
• Document patient progress and communicate changes to the supervising OT.
• Educate patients and caregivers on home programs and safety practices.
• Coordinate services with interdisciplinary team and maintain confidentiality.
