About the Role
Anuco Home Health Care LLC is seeking compassionate, dependable, and highly skilled Occupational Therapists to provide patient-centered rehabilitation services in patients’ homes.
The Occupational Therapist will evaluate patients’ functional abilities, establish individualized plans of care, provide skilled occupational therapy, supervise assigned COTAs and Home Health Aides, and help patients safely perform daily activities in their home environment.
The ideal candidate has Medicare-certified home health experience, strong clinical judgment, proficiency with Axxess Home Health EMR, and a consistent record of accurate and timely documentation.
Responsibilities
- Complete comprehensive occupational therapy evaluations and reassessments.
- Evaluate functional mobility, strength, coordination, cognition, safety awareness, endurance, and ability to perform activities of daily living.
- Develop, implement, evaluate, and revise individualized occupational therapy plans of care.
- Perform skilled occupational therapy procedures according to physician orders and the established plan of care.
- Complete applicable OASIS assessments, recertifications, resumptions of care, transfers, and discharge documentation.
- Develop treatment goals that support safety, independence, and improved performance of daily activities.
- Provide ADL and IADL retraining, therapeutic exercise, home-safety instruction, adaptive-equipment training, and caregiver education.
- Recommend appropriate assistive devices, durable medical equipment, and environmental modifications.
- Communicate patient progress and recommended treatment changes to physicians and the interdisciplinary team.
- Instruct patients and caregivers regarding home programs, safety strategies, and continued care.
- Complete progress reports, recertification documentation, and discharge summaries.
- Supervise assigned COTAs and Home Health Aides according to agency and regulatory requirements.
- Participate in case conferences, staff meetings, medical-record reviews, QAPI activities, and required training.
- Promptly report incidents, complaints, changes in condition, hospitalizations, falls, or unmet needs.
- Protect patient confidentiality and comply with HIPAA and agency policies.
Axxess and Documentation Expectations
- Complete evaluations, visit notes, reassessments, recertifications, and discharge documentation accurately in Axxess.
- Submit all documentation within agency-required timeframes.
- Ensure documentation reflects the skilled service provided, patient response, progress toward goals, and continued need for occupational therapy.
- Confirm that visit frequency, physician orders, goals, interventions, and documentation are consistent.
- Document patient and caregiver education, coordination of care, and communication with physicians and other clinicians.
- Respond promptly to QA corrections and return corrected documentation within the requested timeframe.
- Notify the office immediately if a clinical, scheduling, or technical issue may delay documentation.
- Understand that incomplete or late documentation may delay care coordination, authorization, billing, and future assignments.
Requirements
- Current, unrestricted Illinois Occupational Therapist license.
- Graduate of an accredited Occupational Therapy program.
- NBCOT certification or eligibility as applicable.
- Home health experience strongly preferred.
- OASIS experience preferred.
- Axxess Home Health EMR experience preferred.
- Two years of occupational therapy experience preferred.
- Current CPR or BLS certification.
- Valid driver’s license, reliable transportation, and current automobile insurance.
- Professional liability insurance as required.
- Strong clinical judgment, communication, organization, and time-management skills.
- Ability to work independently and travel to patient homes.
- Ability to perform essential job duties with or without reasonable accommodation.
Clinical Territories
Applicants may select one or more territories they can cover consistently.
North Territory
Des Plaines, Grayslake, Mount Prospect, Palatine, Glendale Heights, Schiller Park, Carol Stream, Norridge, Niles, and north/northwest Chicago.
Territory assignments depend on patient location, clinician availability, payer requirements, clinical competency, and agency needs.

- Provide appropriate supervision and communication to assigned COTAs and HHAs.
- Demonstrate dependable attendance, sound clinical judgment, and professional conduct.
