Job summary

Anuco Home Health Care LLC is seeking compassionate Occupational Therapists to provide patient-centered rehabilitation in patients’ homes. You will evaluate functional abilities, create individualized plans, supervise COTAs and HHAs, and document care with Medicare-guided standards.

Ideal candidates have Medicare home health experience, strong clinical judgment, proficiency with Axxess EMR, and a proven track record of accurate documentation.

Qualifications

• Current, unrestricted Illinois Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• NBCOT certification or eligibility as applicable.
• Home health experience strongly preferred.
• OASIS experience preferred.
• Two years of occupational therapy experience preferred.
• Current CPR or BLS certification.
• Valid driver’s license, reliable transportation, and current automobile insurance.
• Professional liability insurance as required.
• Strong clinical judgment, communication, organization, and time-management skills.
• Ability to work independently and travel to patient homes.

Responsibilities

• Complete comprehensive occupational therapy evaluations and reassessments.
• Evaluate functional mobility, strength, coordination, cognition, safety awareness, endurance, and activities of daily living.
• Develop, implement, evaluate, and revise individualized occupational therapy plans of care.
• Perform skilled occupational therapy procedures according to physician orders and the established plan of care.
• Complete applicable OASIS assessments, recertifications, resumptions of care, transfers, and discharge documentation.
• Develop treatment goals that support safety, independence, and improved performance of daily activities.
• Provide ADL and IADL retraining, therapeutic exercise, home-safety instruction, adaptive-equipment training, and caregiver education.
• Recommend appropriate assistive devices, durable medical equipment, and environmental modifications.
• Communicate patient progress and recommended treatment changes to physicians and the interdisciplinary team.
• Instruct patients and caregivers regarding home programs, safety strategies, and continued care.
• Complete progress reports, recertification documentation, and discharge summaries.
• Supervise assigned COTAs and Home Health Aides according to agency and regulatory requirements.
• Participate in case conferences, staff meetings, medical-record reviews, QAPI activities, and required training.
• Promptly report incidents, complaints, changes in condition, hospitalizations, falls, or unmet needs.
• Protect patient confidentiality and comply with HIPAA and agency policies.
