Job summary

CenterWell Home Health, part of Humana, seeks an Occupational Therapist to help patients regain strength and independence in home settings. You will assess functional ability, plan retraining programs, and work with physicians to implement care plans.

Responsibilities include measuring progress, coordinating with the rehab team, and educating patients and families on exercises and equipment. A valid OT license and CPR are required, with reliable transportation.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient's level of function.
• Confers with the physician and care team and participates in development/revision of the Plan of Care.
• Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, ROM and function.
• Review/expand retraining programs aligning with behavioral goals and rehab guidelines.
• Consult with vocational team members to develop and implement vocationally oriented plans.
• Report patient status and progress to physicians, rehab staff, management, patient and family.
• Coordinate care and discharge planning and support other health care personnel.
