Job summary

Optimal Care is seeking a Home Health Occupational Therapist to work one-on-one with patients in their homes, helping them regain independence in daily activities. You will assess abilities, create personalized plans, and teach adaptive strategies to support safe living at home.

The role requires travel within Marion, Hamilton, Madison and surrounding counties, with hours from Monday to Friday, 8:00 AM to 5:00 PM and occasional weekend rotation.

Qualifications

• Graduate of an ACOTE-accredited OT program.
• Current state license as an OT.
• Minimum 1 year of OT experience.
• Willingness to travel daily for home visits within service area.

Responsibilities

• Assess functional abilities and design personalized home-based treatment plans.
• Provide hands-on training and education to patients and families.
• Collaborate with an interdisciplinary care team to support independence and safety.

ACOTE accredited Occupational Therapy program

Current state OT license

Minimum 1 year experience as an OT
