Job summary

Optimal Care is seeking a Home Health Occupational Therapist to work one-on-one with patients in their homes, helping them regain independence in daily activities such as dressing, bathing, cooking, and routines. You will assess, plan, and implement personalized treatments, educating patients and families while collaborating with the care team.

This role requires a licensed OT, ACOTE-accredited program, and daily travel within the service area to diverse home and facility settings.

Qualifications

• Graduate of an ACOTE-accredited occupational therapy program.
• Current state license as an Occupational Therapist.
• Minimum 1 year of experience as a licensed Occupational Therapist.
• Reliable transportation and valid driver's license.
• Willingness to travel daily for home visits within the service area.

Responsibilities

• Assess functional abilities and design personalized treatment plans.
• Develop adaptive strategies and solutions to support independence and safety.
• Provide education, hands-on training, and encouragement to patients and families.
• Collaborate with interdisciplinary care team to optimize outcomes.
• Travel daily to patients' homes and care settings within service area.
