Job summary

Optimal Care, Inc. seeks a Home Health Occupational Therapist to work one-on-one with patients in their homes, helping them regain independence through functional assessments and personalized plans. You will design adaptive strategies, educate families, and collaborate with a care team to support safe, meaningful daily living.

The role requires travel within the service area and regular hours Monday–Friday with on-call rotations. Experience in home health and a current OT license are essential.

Qualifications

• > Graduate of an ACOTE-accredited occupational therapy program.
• }> Current state license as an Occupational Therapist.
• > Minimum 1 year of experience as a licensed Occupational Therapist.
• > Reliable transportation, valid driver’s license, and automobile insurance.
• > Willingness to travel daily for home visits within service area.

Responsibilities

• Assess functional abilities and daily living skills in patients.
• Design personalized treatment plans to support independence.
• Develop adaptive strategies and home modifications as needed.
• Provide education, hands-on training, and encouragement to patients and families.
• Collaborate with interdisciplinary care team to optimize outcomes.
• Travel daily to patient homes, facilities within the service area.
