Job summary

AllStar Therapies is seeking a compassionate Home Health Occupational Therapist (OT) to provide in-home therapy services, helping patients regain independence while remaining safely in their homes with one-on-one, patient-centered care. The OT will evaluate patients in their homes, develop individualized treatment plans, and deliver skilled therapy in accordance with physician orders.

Qualifications

• Current OT license in the state of practice.
• Graduate of an accredited OT program.
• Home health experience preferred.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently with a flexible daily schedule.
• Excellent communication, documentation, and organizational skills.
• CPR certification preferred.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled occupational therapy services in accordance with physician orders and the established plan of care.
• Assess and improve patients' ability to perform ADLs and IADLs.
• Recommend adaptive equipment, home modifications, and safety strategies.
• Educate patients, families, and caregivers on treatment plans and home exercise programs.
• Complete timely and accurate documentation in accordance with agency, state, federal, and Medicare guidelines.
• Communicate effectively with physicians, nurses, case managers, and the interdisciplinary care team.
• Participate in care conferences and discharge planning as needed.
