Job summary

Adaptive Home Health is seeking an Occupational Therapist (OT) to join our field-based team. This full-time role involves delivering in-home OT visits, evaluating patients, and creating plans focused on ADLs/IADLs, safety, and independence.

You will coordinate with the care team, document thoroughly, and support equipment training and home modifications. The position offers autonomy with a supportive operations team and a productivity-based pay model.

Qualifications

• Active Colorado OT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when condition or needs change.
• Document promptly and thoroughly — defensible, efficient, and consistent.
