Lifespan Therapy is seeking a Home Health Occupational Therapist to perform in-home assessments and develop personalized plans to improve ADLs and home safety. You will schedule visits according to your preferred caseload, collaborate with a supportive team, and help patients reach independence in their own homes.

The role emphasizes flexible, app-based workflow, with credentials management, referrals, and billing handled by the company, enabling clinicians to focus on patient outcomes.

Qualifications

• Valid California Occupational Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test within the last year.
• Professional liability and business license (if applicable).

Responsibilities

• Provide in-home assessments to address ADLs, IADLs, and home safety.
• Develop personalized treatment plans focusing on energy conservation and adaptive equipment.
• Schedule and conduct patient home visits based on preferred caseload.
• Collaborate with a team to ensure holistic care for each patient.
