Responsibilities

• Responds to service requests and initiates referrals when appropriate.
• Assesses patients to obtain data for treatment planning per physician's orders.
• Interprets evaluation findings to patients, families, and care team.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates plans with patient and care team; may supervise assistants.
• Implements treatment directly or supervises therapy provided by assistants.
