CVS Health Corporation, via Signify Health, seeks a Full Time Clinician (NP or PA) to provide In-Home Health Evaluations. You’ll travel across a defined coverage area, up to 100% of the time, delivering visits in the comfort of members’ homes and performing medication and history reviews with an iPad workflow.

The role requires active, unrestricted licenses in the coverage areas, board certification, and multi-state licensure as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses as required.
• Able to cover all counties within state and flex to other areas of business need.
