Signify Health, part of CVS Health, seeks a Full‑Time Clinician (NP or PA) to provide In‑Home Health Evaluations across a defined coverage area. You will travel locally and nationally up to 100% of the time, with potential overnight stays, car rentals or flights as needed, and must hold multiple state licenses as required.

Visits take about 45 minutes, including medication and medical history review, physical exam, and, if ordered by the health plan, additional tests.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Able to obtain and maintain multi-state licenses as needed.
• Able to cover all counties within state and flex to other areas of business need.

Responsabilités

• Provide In-Home Health Evaluations across a defined coverage area.
• Travel locally and within licensed states up to 100% of the time.
• Conduct visits, record medication and medical history, and perform physical evaluations.
• Utilize iPad and clinical workflow; no prescribing or lab orders required.
• Coordinate with Signify Health to connect members with appropriate care.
