Signify Health, part of CVS Health, seeks a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. Travel up to 100% within defined coverage areas, with overnight stays as needed, using iPad and clinical workflow.

Licensure in multiple states will be required. You will conduct 45-minute visits, review meds and history, perform physicals, and coordinate with health plans for additional diagnostics.

Formación

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area.
• Able to cover all counties within state and flex to other areas of business need.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
