Signify Health, part of CVS Health®, is hiring a Full Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations across a defined coverage area. You will travel up to 100% to member homes, perform comprehensive health assessments, and follow the clinical workflow on an iPad.

You must hold active, unrestricted licenses in coverage areas and board certification; multi-state licensing will be required and travel may include overnight stays.

Competenze

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area, as required.
• Able to cover all counties within state and flex to other areas of business need.

Mansioni

• Provide In-Home Health Evaluations with patients.
• Travel locally and across licensed states up to 100%.
• Engage patients in homes and perform health assessments and related services.
• Utilize iPad-based workflow; no prescribing nor lab orders required unless specified.
