Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to perform in-home health evaluations for members in their own homes. Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and selective tests ordered by the health plan.

You’ll use an iPad and a streamlined workflow. Part-time schedule is ~29.5 hours weekly with a pay range of $37.66–$81.11 hourly.

Qualifications

• Active, unrestricted license(s) in coverage areas.
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Bring your heart into every visit, joining a national network of clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide with the goal of connecting to the right care for unique needs.
• Collaborate with Signify Health clinicians to fulfill the role requirements.
• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening).
