Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations, visiting patients in their own homes and bridging gaps in care.

Join a national network of clinicians delivering flexible, comprehensive assessments. Expect about 29.5 hours weekly with travel to licensed states; virtual visits are possible, and the pay range is $41.85–$90.13 per hour.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).

Responsibilities

• Provide in-home health evaluations to patients in their homes.
• Build trusted relationships to improve health outcomes.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Deliver Diagnostic & Preventive Services as indicated.
