Job summary

You will use an iPad with a streamlined workflow; prescribing or altering treatment is not required. Anticipated weekly hours are 29.5, with travel within licensed states and occasional virtual visits.

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)

Responsibilities

• Provide in-home health evaluations for health plan members
• Engage with individuals and review medical history/medications
• Perform physical evaluations and order the required diagnostic tests as directed
• Travel locally within licensed states and conduct virtual visits when needed
• Collaborate with Signify Health team to ensure care quality and proper documentation
