Job summary

Signify Health, part of CVS Health, seeks a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for health plan members.

You will travel locally within licensed states, perform 45-minute visits, review medications and medical history, and use an iPad-based clinical workflow during evaluations. This role emphasizes patient education and care coordination.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (NP/PA).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations for health plan members.
• Visit members nationwide and connect them with the right care.
• Travel locally within licensed states as needed.
• Conduct virtual visits as needed.
• Use iPad and clinical workflow during evaluations.
• Do not prescribe or alter treatments unless ordered by the health plan.
