Job summary

Signify Health, part of CVS Health, seeks a Part-Time Clinician (NP or PA) to perform in-home health evaluations for members in their homes, bridging gaps in care with compassion and accuracy.

You will conduct assessments using iPad workflows, review medications and medical history, and deliver diagnostic and preventive services as ordered by health plans. Local travel and licensed-state coverage required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide In-Home Health Evaluations for health plan members.
• Engage with patients at home and review medical history and medications.
• Perform physical evaluations and, if ordered, diagnostic tests.
• Travel locally and within licensed states; conduct virtual visits as needed.
