Signify Health, Inc., a CVS Health company, seeks full-time Clinicians (NPs or PAs) to perform in-home health evaluations nationwide. You will engage with members in their own homes to review medications and medical histories, conduct physical evaluations, and support connections to the right care.

Travel locally and within licensed states as needed; occasional virtual visits; and a comprehensive benefits package awaits full-time colleagues dedicated to improving health outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.
