Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You will engage with individuals in their homes and help bridge gaps in care.

The role requires active medical licenses, board certification, and willingness to travel locally and within licensed states. You will conduct 45-minute visits, perform health assessments, and use an iPad-based workflow.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
