The role involves visiting patients in their homes, performing reviews of medications and medical history, and conducting physical evaluations with optional diagnostic tests as ordered by health plans.

Anticipated weekly hours are 40, with full-time status and a pay range that reflects experience and geography.

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)

Responsabilités

• Provide in-home health evaluations to patients in their residences
• Engage with patients to bridge gaps in care
• Conduct virtual visits as needed
• Travel locally and within licensed states to conduct visits
• Utilize iPad and clinical workflow for assessments
