Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visits last about 45 minutes and include health history review, a physical evaluation and any ordered diagnostic tests. You’ll use an iPad and a clinical workflow, may travel locally and across licensed states, and conduct virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Perform health screenings, review medical history, and conduct physical assessments.
• Travel locally and within licensed states; conduct virtual visits as needed.
