Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to perform in-home health evaluations for health plan members. You will conduct 45‑minute visits, review medications and histories, perform physicals, and offer preventive diagnostic services, with travel as needed. No prescribing required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsabilités

• Provide In-Home Health Evaluations.
• Engage with patients during home visits and address health questions.

• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Provide preventive and diagnostic services per program guidelines.
