Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (NP or PA) to perform in‑home health evaluations in members’ residences. You will build trusted relationships and bridge gaps in care, delivering personalized, comprehensive assessments at the doorstep.

You will visit members for about 45 minutes, review medications and history, perform a clinical evaluation, and coordinate any plan-ordered tests. Travel locally across licensed states will be required.

Qualifications

• Active, unrestricted license(s) in coverage areas.
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations to members.
• Engage with patients to review medication and medical history.
• Perform physical evaluations and document findings.
• Coordinate with health plans for tests ordered.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
