Responsibilities

• Conduct 45‑minute home visits to assess health needs and review medications.
• Perform physical evaluation and health plan–ordered diagnostics when applicable.
• Complete virtual visits as needed and coordinate care with members’ plans.
• Travel locally within licensed states to meet members where they live.
