Job summary

LiveWell Homecare Agency, LLC is seeking a licensed Nurse Practitioner to provide comprehensive in-home health assessments for adult and geriatric patients. The role involves reviewing medical history, coordinating with physicians and families, and delivering care at homes and SNFs with emphasis on accurate documentation and patient education.

The ideal candidate holds NP licensure with prescriptive authority, board certification in relevant specialties, and at least one year of NP experience,

Qualifications

• Master’s Degree or equivalent experience with NP licensure and prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care.
• Minimum one year NP experience.
• Current BLS, ACLS, or CPR certification.
• Ability to travel extensively, including overnight and weekend stays.
• Valid driver’s license and insurance in good standing.
• Strong communication, computer skills, and familiarity with medical records software.

Responsibilities

• Perform in-home health assessments and document thoroughly following protocols.
• Review medical history, diagnosis, and care plans with providers, social workers, and family members.
• Administer point-of-care and lab testing as needed.
• Coordinate with physicians, case managers, and caregivers to improve care quality.
• Deliver services at members’ residences, including homes and Skilled Nursing Facilities.
