Job summary

Optum Home & Community Care, part of the Optum family, is hiring a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage and other members. This non-prescribing, field-based role focuses on education, gap closure, and collaboration with PCPs.

The role offers flexibility with a 8am–8pm schedule across a 7-day week, no on-call, weekends, or holidays. A $10,000 sign-on bonus is available for external candidates.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the state of practice.
• Active and unrestricted driver’s license; reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care; coordinate with primary care providers.
