Job summary

UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (NP) to perform comprehensive in‑home health assessments for Medicare Advantage members. This field role emphasizes education, gap closure, and collaboration with primary care providers in a supportive, patient‑centered team.

The role requires NP certification, active licensure, and the ability to travel to patient homes. A flexible schedule and strong care coordination skills are valued in this pay‑for‑performance

Qualifications

• Active ANCC or AANP certification for NP role (Family, Adult, Geriatric, or Adult‑Gerontology).
• Active, unrestricted license in practicing state by start date (or ability to obtain).
• Unrestricted driver’s license and reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in‑home health assessments for Medicare Advantage and other plan members.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close gaps in care and coordinate referrals as needed.
