Job summary

UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to conduct comprehensive in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible scheduling, a no on-call model, and a compensation package with base pay plus bonus.

Qualifications

• Active NP or PA certification varying by role (ANCC/AANP for NP, NCCPA for PA).
• Current, unrestricted licensure in practicing state.
• Active driver's license and reliable transportation for home visits.

Responsibilities

• Conduct in-home assessments and vital signs, med reconciliation, and physical exams.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close care gaps.
• Identify urgent situations and intervene as needed.
• Educate patients on disease processes, medications, and adherence.
