UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role prioritizes education, gap closure, and collaboration with PCPs to improve health outcomes.

Requirements include active NP/PA certification, state licensure, valid driver’s license, and ability to work in varied home environments. Flexible scheduling supports work-life balance and excludes weekends and holidays.

Formación

• Active NP/PA certification in relevant family/primary care track.
• State licensure in practicing state.
• Driver’s license and reliable transportation for home visits.
• Fluency in languages besides English is a plus.

Responsabilidades

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to address care gaps.
• Educate members on disease processes, medications, and adherence.
