OptumCare HouseCalls is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plans. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes, within a flexible 20–40 hour workweek and no weekends or holidays.

You will join an interdisciplinary team delivering preventive care at patients' homes and connecting them to needed services across settings.

Qualifications

• Must have ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted Colorado licensure or ability to obtain by start date with no disciplinary actions.
• Active and unrestricted driver's license with reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments (sit/stand/kneel as needed).
• Preferred: 1 year of clinical experience; proficiency with EMR; ability to transport equipment up to 30 pounds and navigate stairs; strong communication with geriatric/Medicare populations.

Responsabilités

• Conduct comprehensive in-home assessments including medical history, medications, vital signs, and physical exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
