Job summary

Optum Home & Community Care is seeking Advanced Practice Clinicians (Nurse Practitioner or Physician Assistant) to deliver care in patients' homes across the assigned territory, with no evenings, weekends, or holiday coverage. This field-based role emphasizes building meaningful patient relationships and maintaining a healthy work-life balance.

You will join a collaborative care team, perform in-home assessments, manage acute and chronic conditions, document care, and lead goals-of-care

Responsibilities

• Perform comprehensive in-home assessments including medical history, medication reconciliation, physical examinations and vital signs.
• Manage patients with acute illnesses or exacerbations of chronic conditions through assessment, diagnostics, and treatment interventions.
• Support members during short-term rehabilitation and recovery periods.
• Maintain accurate and timely clinical documentation, including ICD-10 coding.
• Collaborate with physicians, nurses, care coordinators, and other interdisciplinary team members.
• Lead advanced illness and goals-of-care discussions with patients and families.
• Provide education, counseling, and support to patients and caregivers.
