UnitedHealth Group is seeking a Nurse Practitioner or Physician Assistant for in‑home care across a designated area. This full-time role includes daytime in‑home assessments, care planning, and collaboration with an interdisciplinary team to improve patient outcomes while supporting independent care delivery.

You’ll travel to patient homes daily, with no night, weekend, or holiday commitments. The position offers a competitive compensation package and a strong benefits program as part of Optum

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care
• For PAs: NCCPA certification and state licensure
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions
• Active and unrestricted driver’s license with reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post‑hire
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties

Responsabilités

• Conduct comprehensive in-home assessments and vital signs with relevant testing
• Manage members through acute or chronic condition scenarios
• Oversee short-term sub-acute rehabilitation episodes
• Ensure accurate documentation including ICD-10
• Collaborate with interdisciplinary care team
• Conduct advanced illness and end-of-life conversations
• Provide counseling and education to patients and caregivers
