Qualifications

• Active state NP license.
• Current NP National Board Certification.
• Active state DEA licensure.
• Master’s degree in Nursing (MSN).
• Five or more years of nursing experience; at least two years of NP experience preferred.
• Strong interpersonal communication skills — verbal and written.
• Excellent time management skills with accountability for daily workflow.
• Proficiency in medical instruments and equipment.
• Strong knowledge base of HIPAA and OSHA regulations.
• Proficiency in computer-based data management programs, EMR systems, and Microsoft Office.

Responsibilities

• Provides primary care in home settings (skilled nursing, assisted living, independent living).
• Performs clinical assessments and develops personalized care plans.
• Collaborates with interdisciplinary teams for seamless care transitions.
• Manages chronic conditions and acute visits with a focus on prevention.
• Provides referrals to other healthcare practitioners when necessary.
• Keeps patient medical records up to date.
