Abode Care Partners is seeking a compassionate Nurse Practitioner to deliver primary care to seniors and patients with complex needs in their homes across various housing settings. You will perform clinical assessments, develop personalized care plans, and collaborate with interdisciplinary teams to improve outcomes while managing chronic conditions and acute visits with a preventative focus.

Qualifications

• Active NP license in the relevant state.
• Current NP national board certification.
• Active DEA licensure.
• MSN degree required.
• Five+ years of nursing experience; NP experience preferred.
• Strong communication skills, both verbal and written.
• Excellent time management with daily workflow accountability.
• Proficiency with medical instruments and equipment.
• Knowledge of HIPAA and OSHA regulations.

Responsibilities

• Plan, direct, and evaluate patient medical needs with evidence-based care.
• Treat chronic, urgent, and minor injuries, and other illnesses.
• Order and interpret labs and imaging as needed.
• Collaborate with caregivers and healthcare professionals to maintain a care plan.
• Provide referrals to other practitioners when necessary.
• Maintain up-to-date medical records and documentation.
