Abode Care Partners seeks a compassionate, autonomous Nurse Practitioner to provide primary care to seniors and complex patients in their homes across Nashville. The role emphasizes comprehensive home-based care and collaboration with interdisciplinary teams.

Formación

• Active state NP license
• Current NP national board certification
• Active state DEA license
• MSN degree required
• 5+ years nursing experience; at least 2 years NP experience preferred
• Strong interpersonal and written communication
• Excellent time management and accountability
• Proficient with medical instruments and equipment
• Knowledge of HIPAA/OSHA regulations
• Experience with EMR and MS Office
• Valid driver’s license and auto insurance
• Ability to lift up to 25 lbs and stand/walk for extended periods

Responsabilidades

• Performs evidence-based medical care and plans patient needs
• Examines chronic and urgent illnesses, minor injuries and injuries
• Orders and interprets labs and tests as needed
• Collaborates with caregivers and healthcare professionals for care plans
• Provides referrals when necessary
• Keeps medical records up to date
