Job summary

Abode Care Partners in Tennessee is seeking a compassionate, autonomous Nurse Practitioner to deliver personalized primary care to seniors and clients with complex needs in their homes.

You will perform assessments, develop care plans, manage chronic conditions, and collaborate with interdisciplinary teams to ensure smooth transitions and preventive care, with opportunities for performance incentives and comprehensive benefits.

Qualifications

• Active state NP license.
• Current NP National Board Certification.
• Active state DEA licensure.
• MSN degree or higher.
• Five or more years of nursing experience; at least two years NP experience preferred.
• Strong verbal and written communication.
• Excellent time management and accountability for daily workflow.
• Proficient with medical instruments and equipment.
• Knowledge of HIPAA and OSHA regulations.
• Experience with EMR and data management systems.
• Licensed driver with valid auto insurance.
• Ability to engage in moderate physical activity.

Responsibilities

• Plans, directs, and evaluates patient medical needs.
• Examines chronic and urgent illnesses, injuries.
• Orders and interprets labs and diagnostic tests.
• Collaborates with caregivers and other professionals.
• Provides referrals to other practitioners when needed.
• Keeps patient records up to date.
