Job summary
Abode Care Partners is seeking a compassionate Nurse Practitioner to provide comprehensive primary care to seniors and patients with complex needs in their homes across Tennessee, including Cleveland. You will conduct clinical assessments, develop personalized care plans, and collaborate with interdisciplinary teams to improve outcomes.

Qualifications
- Active state Nurse Practitioner license (TN)
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master’s degree in Nursing (MSN)
- Five+ years of nursing experience; two+ years NP experience preferred
Responsibilities
- Performs primary care assessments and develops personalized care plans for seniors in home and community settings
- Treats chronic and acute conditions in skilled nursing, assisted living, and independent living environments
- Orders and interprets laboratory and diagnostic tests as needed
- Collaborates with interdisciplinary teams to ensure seamless care transitions
- Maintains up-to-date medical records and coordinates referrals
