Job summary

Optum Home & Community Care is seeking a field-based clinician to perform in-home preventive care assessments for Medicare Advantage and other health plan members. The role emphasizes closing care gaps, improving outcomes, and collaborating with primary care providers, offering strong work-life balance and scheduling flexibility.

The position requires NP or PA certification with state licensure, active driver's license, and willingness to travel for home visits.

Qualifications

• Active, unrestricted state license (or ability to obtain before start date)
• Active, unrestricted driver's license and reliable transportation for home visits
• Ability to perform assessments in varied home environments and navigate physical requirements of the role
• Fluency in additional languages is valued and may come with additional compensation

Responsibilities

• Conduct comprehensive in-home preventive care assessments and close care gaps
• Identify care gaps and improve health outcomes for Medicare Advantage and other health plan members
• Collaborate with primary care providers to coordinate care and improve member experience
• Provide scheduling flexibility with field-based care delivery

Education

ANCC/AANP Family/Adult/Geriatric Primary Care NP certification or NCCPA PA certification and state licensure
