Job summary

Optum is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage members. This field-based, non-prescribing role focuses on preventive care and patient education, collaborating with primary care providers to improve health outcomes.

Responsibilities include in-home assessments, point-of-care testing, and addressing social determinants of health.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted licensure in the practicing state with no disciplinary actions.
• Active and unrestricted driver's license or ability to obtain it by start date.

Responsibilities

• Conduct comprehensive in-home health assessments.
• Perform point of care testing.
• Identify and close gaps in care.
• Educate patients on disease management, medications, and preventive care.
• Address social determinants of health and connect members with resources.
• Work independently in the field, supported by an interdisciplinary team.
