Qualifications

• Active, unrestricted NP/PA license or ability to obtain by start date.
• ANCC or AANP certification for NP or NCCPA certification for PA.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plans.
• Review medical history and medications.
• Perform physical exams, screenings, and point-of-care testing.

Education

ANCC or AANP certification (Family/Adult/Geriatric)

NCCPA certification and active state licensure
