Job summary

Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This non-prescribing, field-based role focuses on preventive care, health education, identifying care gaps, and partnering with primary care providers to improve patient outcomes.

Compensation includes base pay and bonus with a sign-on option and strong work-life balance.

Qualifications

• ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted license (or ability to obtain by start date).
• Nurse Practitioner or Physician Assistant credentials with state licensure and transportation ability.

Responsibilities

• Conduct comprehensive in-home health assessments.
• Review medical history and medications.
• Perform physical exams, screenings, and point-of-care testing.
• Identify and document diagnoses for care management and treatment planning.

Education

ANCC or AANP Family/Adult/Geriatric Primary Care certification

Active state licensure for NP/PA

Driver's license and reliable transportation
