Job summary

UnitedHealth Group’s HouseCalls program is seeking a licensed Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers.

The position offers a flexible, home-based schedule with no on-call, weekends, or holidays, and includes a comprehensive benefits package and bonus opportunities contingent on performance.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active licensure in the practicing state without disciplinary actions.
• Active and unrestricted driver's license with access to reliable transportation.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments including history, vital signs, and physical examination.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
