Job summary

UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers, without prescribing authority.

The position requires active licensure, relevant certifications, and reliable transportation for home visits, with a flexible schedule across 8am–8pm or 7-day weeks.

Qualifications

• 1+ years of clinical experience (family, geriatric, or home health preferred).
• Proficiency with electronic medical records and technology.
• Proven communication skills with geriatric or Medicare populations.

Responsibilities

• Conduct comprehensive in-home assessments, including reviewing medical history, medications, vitals and physical exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
