Job summary

UnitedHealth Group is seeking an in-home Advanced Practice Clinician (NP or PA) to conduct annual health assessments for Medicare Advantage members. You will work in HouseCalls, delivering care directly at patients’ homes and collaborating with primary care providers to improve outcomes.

The role emphasizes education, gap closure, and non-prescribing field-based care with flexible scheduling (8am–8pm, 7 days). A driver’s license and state licensure are required.

Qualifications

Active licensure in the state of practice with no disciplinary actions.
For NPs: ANCC or AANP certification in Family/Adult or Adult-Geriatric.
For PAs: NCCPA certification and state licensure.

Responsibilities

Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
Document diagnoses for care management and treatment planning.
Communicate findings to members’ PCPs to close care gaps.
Identify urgent/emergent situations and intervene appropriately.
Educate members on diseases, medications and adherence.
Address social determinants of health and provide referrals.
