Job summary

UnitedHealth Group is seeking a Nurse Practitioner or Physician Assistant to join the House Calls team in Snohomish County, WA. This field-based role conducts in-home health assessments for Medicare Advantage and other members, collaborating with PCPs to improve outcomes and close care gaps.

Responsibilities include physical exams, med reconciliation, patient education, and addressing social determinants of health. Flexible scheduling supports 24–40 hours/week with no on-call requirements.

Qualifications

• For NP: active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PA: current NCCPA certification and state licensure
• Active, unrestricted licensure in Washington or ability to obtain by start date

Responsibilities

• Conduct comprehensive in-home assessments including medical history, meds reconciliation, vitals and exams
• Perform screenings and point-of-care testing as appropriate
• Document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to close care gaps
• Identify urgent situations and intervene appropriately
• Educate members on disease processes, medications and adherence
• Address social determinants of health and provide referrals
• Flexible scheduling between 8 a.m. and 8 p.m. over a 7-day week
• No on-call, weekends or holidays required
